If your index.shtml has something like:

<!--#include virtual="/cgi-bin/foo.pl" -->

This will fail to be included if you go to http://www.domain.com/ but it will work if you go to http://www.domain.com/index.shtml. To fix this place a .htaccess in the html folder that has:

RedirectMatch ^/$ http://www.domain.com/index.shtml

This will force a redirection to the working URL. This is being fixed in a future release.
