Error: phpMyAdmin tried to connect to the MySQL server, and the server rejected the connection

When trying to access their WebAdmin / PHPMyAdmin from the Database Servers page in Parallels Plesk Panel  we receive the following error:
phpMyAdmin tried to connect to the MySQL server, and the server rejected the connection. You should check the host, username and password in your configuration and make sure that they correspond to the information given by the administrator of the MySQL server.
Overall WebAdmin / phpMyAdmin appears to be having issues connecting to the MySQL databases properly.
The following error may also be seen:
#1045 - Access denied for user 'ODBC'@'localhost' (using password: NO)


The issue may be caused by URL rewrite rules misconfiguration in IIS.


Open IIS Manager and go to
<SERVER_NAME> -> URL Rewrite -> <URL_NAME>
Disable any suspicious server level rules. 
It is recommended to create rules on site level rather than on the IIS server level.
